Intégration de l’approche palliative à l’unité de soins intensifs (USI) : étude qualitative des besoins et solutions de l’équipe

Résumé

Problématique : L’approche palliative intégrée constitue désormais une composante essentielle des soins intensifs. Si les obstacles et conditions facilitant l’intégration de cette approche ont été identifiés, les besoins des soignants sont moins connus. 

But : Comprendre la situation actuelle, identifier les besoins et solutions afin de développer dans une phase subséquente, une intervention visant à intégrer l’approche palliative à l’unité des soins intensifs (USI). 

Méthode : Un devis de recherche-action a été retenu pour cette étude réalisée auprès de 17 membres de l’équipe soignante d’une USI québécoise. Des entrevues individuelles, dont les déclencheurs étaient des situations cliniques, illustrant des conditions facilitant l’intégration de l’approche palliative à l’USI, ont été utilisées comme principal outil de collecte. 

Résultats : Parmi les besoins et solutions à privilégier soulignons que le développement d’une culture palliative interne, l’identification de la clientèle cible, la mise en place d’une activité de formation collective et des moments d’échange faciliterait la vision commune. De son côté, l’adoption d’un processus de décision concertée serait favorisée par un fonctionnement interdisciplinaire, le rehaussement du rôle de l’infirmière et de la documentation. Finalement, un environnement propice serait créé par l’humanisation de l’environnement de soins, le maintien des tournées médicales, des changements à l’organisation des soins et l’offre de ressources. 

Retombées : Par son processus inductif, cette étude permet de guider le développement d’une intervention à plusieurs composantes, susceptibles d’atténuer les obstacles et de répondre aux besoins locaux, puisqu’elles ont été générées par les acteurs terrain locaux eux-mêmes.

Abstract 

Integrating palliative approach in the intensive care unit (ICU): A qualitative study of the team's needs and solutions  

Introduction: The integration of a palliative approach is now recognized as an essential component of intensive care unit (ICU). While the barriers and facilitators have been identified, the needs of caregivers are less well known. 

Objectives: To understand the current situation, to identify needs and solutions in order to develop an intervention to integrate the palliative approach in ICU.

Method: An action research inquiry was chosen for this study. 17 members of an ICU health care team in Quebec were recruited. Real life clinical situations, illustrating facilitating conditions to integrate a palliative approach into the ICU, were used as the primary collection tool for the individual interviews.

Results: The adoption of an integrative palliative care model, locally defined triggers to identify patients needing palliative care, collective educational activity and a unit-based support system are among the needs and solutions identified by the ICU team to promote a common vision. The adoption of an interdisciplinary team approach, an enhanced role of the nurses and increase quality of documentation, are considered among the strategies likely improved to collaborative decision-making process. Finally, creating a caring physical and organisational environment, maintaining physician’s rounds and providing support and resources to grieving relatives are suggested. 

Conclusion: Through its inductive process, this study offers a precious guide to develop a multi-component intervention that can reduce barriers and meet local needs, as they were generated by local actors themselves.
